How can I get my Social Insurance Card (SIN) replaced?

 

Applications are available at my constituency office in Surrey at 201-15957 84th Avenue in Surrey, at HRSDC offices and on the federal government's website by going to http://search.hrsdc.gc.ca.

 

The federal government will charge you $10 to replace your card. It's faster and more convenient for you to apply in person at an HRSDC office (e.g. Surrey), and you won't have to part with your valuable identification documents.

 

Your card should be received by mail within three weeks from the date you submit your application, providing your application meets all relevant criteria.

 
 
How do I file for Employment insurance and how long does it take for my application to be approved?

 

Applications are available at our constituency offices, on-line by going to http://www.hrsdc.gc.ca/en/ei/publications/process.shtml or at any HRSDC office. It takes 4-6 weeks for an application to be processed with all necessary documents, including your Record of Employment (ROE) from your last employer.

 

You'll also need your Social Insurance Number (SIN), personal identification such as your driver's license and your complete bank information.

 

You can apply for EI benefits if you've paid into the EI account and you are unemployed. You can receive regular benefits, maternity, parental and/or sickness benefits, fishing benefits and compassionate care benefits.

 

 
How do I get a Birth, Marriage or Death Certificate?

 

Birth, marriage or death certificates are issued by the provincial government's Vital Statistics Agency.  In the case of births, deaths or marriages which took place in British Columbia, further information is available by going to http://www.vs.gov.bc.ca/?SMSESSION=NO

 

 

If your wallet or purse is stolen, you will need a birth certificate before you can apply for other documents, such as a Social Insurance Card, Health Card or Driver's License.

 

 
How do I obtain a Permanent Resident (PR) Card?

 

The Permanent Residence Card replaced the IMM 1000 record of landing document for travel purposes. Applications are available at our constituency offices.  

   

Individuals must have a PR Card before they can apply for Canadian Citizenship.

I have an outstanding Loan with the Canada Student Loan Program.  What should I do?

 

The most important thing you can do is to try and reduce your debt before the loan is sent to a Collections Agency. When that happens, there is very little an MP's office can do to assist a constituent.

 

Some key telephone numbers to keep in mind: 1-888-819-2516 for the National Student Loan Centre; 1-888-815-4514 if a student has attended a public post-secondary institution and 1-888-587-7452 for a private post-secondary school.

If I disagree with my Income Tax Return, What should I do?

 

First, discuss the situation with the person who helped prepare your income taxes.

 

Second, contact the closest Canada Revenue Agency office near you to discuss your file with a representative.

 

Third, if there is still disagreement, you may wish to engage the services of a tax accountant or tax lawyer to assist you with your dispute. 

 

You may also call our constituency offices and we will make an inquiry on your behalf with CRA.  Please note that we are unable to provide you with any ongoing tax advice or an assessment of the merits of your claim as we are not qualified to provide such advice.

If I am injured on the job, is there a government program that can assist me?

 

Most workplace injuries are covered by provincial workers compensation programs.  Please contact your provincial MLA or check out the information available at:

I’m a senior.  What benefits are there for me?

 

Pensions are available through the Income Security Program, including Old Age Security (OAS) and Canada Pension Plan (CPP). The government also provides Disability and Survivors Benefits through CPP, while a Disability Tax Credit is available through the Canada Revenue Agency (CRA).  Further information is available at

Where can I drop off a completed passport application?

 

As of April 4, 2007, passport applications can be dropped off at the passport receiving office of Service Canada located at Suite 900 13401 108th Avenue, Surrey BC.  Office hours are from Mon-Fri between 8:00am and 4:30pm.

Why didn’t my Canadian Child Tax Benefit (CCTB) or GST Cheque come this month?

 

Child Tax Benefit cheques are issued on the 20th of every month. If your cheque hasn't arrived after 5 business days of that date, please call the toll-free number on your CCTB forms or contact our constituency office.

 

The government issues GST cheques quarterly on the 5th of every month. Cheques can be delayed because of a change of address or arrears with Revenue Canada. Contact the toll-free number on your GST form or call our office for assistance.
